Dear Parents & Guardians,
Benton County Public Health is offering another vaccination clinic for H1N1 influenza at Vinton Shellsburg High School on December 11, 2009 . If you would like your child to receive the vaccine return a signed consent to the school nurse. Consent forms are available in the school office or on the school district website.
On the day of the clinic, your child needs to be feeling well in order to receive the vaccine.
We must have a signed consent for your child to receive the H1N1 vaccine.
